Open Enrollment

Each year all eligible employees are afforded the opportunity to make changes to their employee benefit plan selections. Benefit changes made during the open enrollment period become effective on July 1.
 
During Open Enrollment-
  • During open enrollment, all eligible employees and their dependents that previously declined any of the medical, dental, and prescription benefits can enroll at this time.
  • All current participants already enrolled can make changes to their current plan selections and change to any plan being offered.
  • Anyone wishing to add a dependent, or terminate coverage for themselves or a dependent would need to do so at this time by completing an enrollment/change form.  

Anyone who is making a change will need to complete an enrollment/change form. Please visit the Forms tab for an enrollment/change forms.

Open Enrollment Notices:
 
Lifetime Limit No Longer Applies and Enrollment Opportunity
The lifetime limit on the dollar value of benefits under Cinnaminson Board of Education's medical plans no longer applies. Individuals whose coverage ended by reason of reaching a lifetime limit under the plan are eligible to enroll in the plan from April 1, 2011 through April 30, 2011. For more information contact your Benefits Administrator or your Insurance Broker, Gallagher Benefit Services, at (856) 797-3500.  
 

Opportunity to Enroll in Connection with Extension of Dependent Coverage to Age 26-  Individuals whose coverage ended, or who were denied coverage (or were not eligible for coverage), because the availability of dependent coverage of children ended before attainment of age 26 are eligible to enroll in the Cinnaminson Board of Education medical and prescription health plans. Individuals may request enrollment for such children for 30 days from April 1, 2011 through April 30, 2011. Enrollment will be effective July 1, 2011.  For more information contact your Benefits Administrator or your Insurance Broker, Gallagher Benefit Services, at (856) 797-3500.

  

Designation of Primary Care Providers by Participants or Beneficiaries:

The Cinnaminson Board of Education Aetna Patriot V , Patriot X, and Premier plans require the designation of a primary care provider for participating provider services. You have the right to designate any primary care provider who participates in our network and who is available to accept you or your family members. Until you make this designation, the network carrier may designate one for you. For information on how to select a primary care provider, and for a list of the participating primary care providers, contact Aetna Member Services or log onto www.aetna.com.  For further assistance, please contact our insurance broker, Gallagher Benefit Services, at (856) 797-3500.

 

For children, you may designate a pediatrician as the primary care provider.  

 

Coverage for Obstetric or Gynecological Care:

You do not need prior authorization from our health insurance carrier or from any other person (including a primary care provider) in order to obtain access to obstetrical or gynecological care from a health care professional in our network who specializes in obstetrics or gynecology.  The health care professional, however, may be required to comply with certain procedures, including obtaining prior authorization for certain services, following a pre-approved treatment plan, or procedures for making referrals.  For a list of participating health care professionals who specialize in obstetrics or gynecology, contact Aetna Member Services or log onto www.aetna.com.  For further assistance, please contact our insurance broker, Gallagher Benefit Services, at (856) 797-3500.
